What is Supportive Care with Empassion?

We collaborate with existing care teams, including PCPs and Specialists, to provide an extra layer of care. This care is delivered by specially trained, local providers in the patients home (wherever the patient calls home) and via telephonic visits. Care activities address unique patient, family, and caregiver needs and include:

Physical & Emotional Symptoms

Assessment at each visit and additions to medication regimen as indicated

Home Safety & Fall Risk

Evaluate needs and order DME, PT/OT, when appropriate

Cultural / Spiritual Needs

Chaplain support, as needed

Medication Review & Reconciliation

Assessment and education to support adherence and ensure all necessary medications are in the home

Social Determinants of Health

Identify risks, resource needs, and address via referrals to best resource

Functional Status

Regular assessment, including PPS captured at every visit

Understanding of Illness

Education about disease progression

Caregiver Needs

Support & education

Advance Care Planning

Goals of care discussions and completion of POLST/POST & medical POA forms

Empassion is committed to providing accessible, quality care across the US. For 2024, our network covers 45 states and serves 25,000 lives. We carefully vet our palliative and hospice partners to ensure they meet our high standards and align with our performance goals.
